低氧环境对体内成熟失败人卵母细胞体外
成熟结果的影响

摘要:目的 探讨低氧环境(5% O
2
)对控制性超促排卵周期成熟失败卵母细胞体外成熟、
受精和胚胎发育的影响。方法 共收集 163 个 ICSI 周期中成熟失败卵母细胞 444 枚,随机分
为两组:研究组 237 枚未成熟卵母细胞(GV 期 131 枚,MI 期 106 枚);对照组 207(GV
期 124 枚,MI 期 83 枚)。两组总 GV 期卵母细胞 255 枚,总 MI 期卵母细胞 189 枚。经体
外培养成熟后,根据卵母细胞排出的第一极体形态和卵周隙大小适时行 ICSI,16-18h 观察10
受精,挑出正常受精卵继续培养至扩张囊胚。期间观察和记录卵母细胞成熟、受精及各阶段
胚胎发育情况。结果 (1)255 枚 GV 期卵母细胞 IVM 后共成熟 183 枚,成熟率 71.8%,受
精率 89.6%,双原核(2PN)率 79.2%,2PN 卵裂率 94.5%, D2-4 细胞率 39.4%,D3-8 细
胞率 26.3%,D3≥6 细胞率 47.4%,D3-优质胚胎率 29.2%,囊胚率:21.9%,优质囊胚率 16.7%。
研究组 D3-8 细胞率、D3-优质胚胎率、囊胚率显著高于对照组(
P<0.05),其中 D3-优质胚15
胎率和囊胚率差异极显著(P<0.01),两组间成熟率、受精率、双原核(2PN)率、2PN 卵
裂率、D2-4 细胞率、D3≥6 细胞率、优质囊胚率差异无显著统计学意义(P>0.05)(2)两
组中共 189 枚 MI 期卵母细胞 IVM 后共成熟 175 枚,成熟率 92.6%,ICSI 后受精率 90.3%,
双原核(2PN)率 81.1%,2PN 卵裂率 91.5%, D2-4 细胞率 50.0%,D3-8 细胞率 29.2%,D3
≥6 细胞率 54.6%,D3-优质胚胎率 25.4%,囊胚率 17.7%,优质囊胚率 34.8%。研究组中,20
受精率、双原核(2PN)率显著高于对照组(P<0.05),两组间成熟率、2PN 卵裂率、D2-4
细胞率、D3-8 细胞率、D3≥6 细胞率、D3-优质胚胎率、囊胚率、优质囊胚率差异无显著统
计学意义(P>0.05)。结论 低氧环境(5%O
2
)能够提高控制性超促排卵周期人体内成熟失
败卵母细胞体外成熟后的受精和早期胚胎发育。

Effect of low oxygen on the in-vitro-maturation outcome of
human in-vivo failed-matured oocyte

Abstract: Objective To study the effect of low oxygen on human in-vivo failed-matured oocytes
in vitro maturation, fertilization and embryonic development in controlled ovarian
hyperstimulation cycles.Methods A total of 444 failed-matured oocytes (GV and MI stage) 35
derived from 163patients who underwent ICSI cycles were randomly divided into two groups: the
test group which had 237 in-vivo failed matured oocytes containing 131 GV stage oocytes and 106
MI stage oocytes; the control group which had 207 in-vivo failed matured oocytes containing 124
GV stage oocytes and 83 MI stage oocytes. ICSI time was definited by the morphology of first
polar body extruding from oocytes and the size of perivitelline space.The second polar body and 40
pronuclei were observed 16-18 hours later after ICSI and two pronuclei were cultured until
expanded blastosyst stage. The number of matured oocyte, zygote, two pronucleus (2PN), 2PN
cleavage embryo, day 2 4-cell embryo, day 3 8-cell embryo, day 3 over 6-cell embryo, day 3
high-quality embryo, blastocyst and high-quality blastocyst were recorded during culture
respectively in different groups.Results (1) A total of 183 matured oocytes from 255 GV stage 45
oocytes followed by IVM in two groups. The maturation rate, fertilization rate, 2PN rate, 2PN
