右锁骨下动脉内中膜厚度联合脂蛋白a预测

【摘要】目的 分析右锁骨下动脉内中膜厚度(IMT)、脂蛋白a(Lp-a)与冠状动脉粥样硬化性心
脏病(冠心病,CAD)患者冠状动脉狭窄程度的相关性以及二者联合对CAD患者冠状动脉狭窄程度预测
的准确性。方法 选取2016年10月~2017年10月于徐州医科大学附属医院就诊的疑似冠心病患者共287例,
根据冠状动脉造影结果进行分组,其中非CAD组137例,CAD组150例,收集其年龄、性别、体质指数
(BMI)、糖尿病史、高血压病史等临床基本资料。测量右锁骨下动脉IMT、颈动脉IMT。抽取入选患者的
静脉血,检测Lp-a等指标。根据冠状动脉造影结果,对CAD组患者计算Gensini评分,以47.5分为界,分为
高分组和低分组,比较两组一般临床资料。应用ROC曲线比较颈动脉IMT和右锁骨下动脉IMT及联合Lp-a
对冠状动脉狭窄程度预测的准确性。通过Pearson相关及多重线性回归分析,评估上述临床资料与Gensini评
分的相关程度。结果 CAD组患者右锁骨下动脉IMT、Lp-a明显高于非CAD组(

<0.05);Gensini评分高
分组右锁骨下动脉IMT、Lp-a显著高于低分组(

<0.05)。分别以颈动脉IMT和右锁骨下动脉IMT来预测
CAD患者冠状动脉狭窄程度,AUC(颈动脉IMT)=0.725,AUC(右锁骨下动脉IMT)=0.836;分别以右锁
骨下动脉IMT和右锁骨下动脉IMT联合Lp-a来预测CAD患者冠状动脉狭窄程度,AUC(右锁骨下动脉IMT)
=0.836,AUC(右锁骨下动脉IMT+Lp-a)=0.890;Pearson相关分析显示,Gensini评分与右锁骨下动脉IMT、
Lp-a显著相关,相关系数(

Abstract

Objective
To analyze the correlation between right subclavian artery intima-media thickness
(IMT), lipoprotein a (Lp-a) and the degree of coronary artery stenosis in patients with coronary artery disease (CAD),
and their accuracy in the prediction of coronary artery stenosis in CAD patients.
Methods
A total of 287 patients
with suspected coronary heart disease who were present in our hospital from October 2016 to October 2017 were
selected and grouped according to coronary angiography results. Among them, 137 were in non-CAD group and
150 were in CAD group. Their age, gender, BMI, history of diabetes, history of diabetes, history of hypertension,
and other basic clinical data were recorded. The right subclavian artery IMT and carotid IMT were measured with
a color doppler meter. Venous blood was collected from the selected patients, and indicators such as Lp-a were
detected. According to the results of coronary angiography, patients in CAD group were divided into high group and
low group according to Gensini scores. The general clinical data were compared between the two groups. The ROC
curve was used to compare the accuracy of predicting coronary artery stenosis in carotid IMT and right subclavian
IMT. Pearson correlation analysis was used to calculate the correlation coefficients between the above clinical data
and Gensini scores. Statistically significant indicators were included in the multiple linear regression analysis to
calculate the OR value and used as a regression equation. ROC curve analysis was used to compare the accuracy
of right subclavian artery IMT and right subclavian artery IMT combined with lipoprotein a to predict the degree of
coronary artery stenosis.
Results
In the CAD group, IMT and Lp-a of the right subclavian artery were significantly
higher than those of the non-CAD group (

<0.05); the high subgroup of the right subclavian artery IMT and Lp-a
was significantly higher than that of the low group (
